INSURANCE AND FEES Our
fees are comparable
with those charged by other hematologist/oncologists
in Southeastern Michigan.
We accept cash, checks, Visa and MasterCard.
We
participate with the following insurance plans:
If
you belong
to
a health maintenance organization (HMO),
you
must have a referral from your primary physician.
Most
preferred provider organizations (PPOs) do not require referrals.
However, if AAHOA is not in your network, you must have a referral
and you may be responsible for a co-pay.
It
is your responsibility to be sure that referral authorizations are updated
as necessary. If you do not
have an authorization at the time of your appointment, we will ask you
to reschedule your visit or sign a waiver accepting full responsibility
for the charges. Co-pays
are expected at the time of service. If
you have
commercial insurance or if you are
insured by a plan in which we do not participate, we will expect payment
at the time of service. Our
office will bill your
insurer. If we receive payment
for the visit, we will reimburse you.
If there is a difference between the fees we charge and the amount
your insurance pays,
you will be responsible for the balance and for any deductibles.
Our insurance
department (734-712-1010)
will answer questions regarding bills or insurance coverage.
Limited
financial assistance
is
available to qualified patients through St. Joseph Mercy Hospital, the
Leukemia Society of America, American Cancer Society, pharmaceutical companies,
and other agencies. These
resources may provide reimbursement for travel expenses, medications,
and other uncovered medical expenses.
